EMPLOYEE INFORMATION

 

Please notify Human Resources of any of the following changes through Employee Self Service

  • Address
  • Phone number
  • Emergency contacts
  • Tax status 

Please submit an Employee Information Change Form with required documentation for:

  • Driver’s License updates
  • Marital Status Changes  (require copies of marriage license, divorce decree, etc.)
